Patients come to us frustrated after seeing their primary care provider and being told something is “wrong” with their thyroid labs — usually because their TSH is suppressed. In this video, Lindsay Burgess, Nurse Practitioner and hormone specialist, explains why this happens and why relying on TSH alone to manage thyroid treatment can be misleading. This issue comes up constantly with patients who are treated with natural desiccated thyroid (NDT) or medications that contain T3, the active thyroid hormone. When patients receive adequate thyroid hormone and begin to feel better, the body’s feedback system often suppresses TSH. This is a normal physiological response. However, many providers are trained to view any suppressed TSH as dangerous or a sign of overtreatment — even when patients are feeling significantly better. At our clinic, thyroid treatment is not based on TSH alone. We evaluate a combination of: • Patient symptoms • Free T3 levels (the active hormone) • Free T4 levels • Overall clinical response Many patients continue to struggle with fatigue, brain fog, weight gain, and low energy when thyroid treatment is managed solely by TSH. When therapy is optimized using Free T3 and symptom response, patients often experience significant improvements in energy, metabolism, mood, and cognitive function. So how do we know this approach works? Because we see it every day. Patients who struggled for years often feel dramatically better once thyroid treatment is optimized properly. Short Science Section (Concise) TSH is regulated through a negative feedback loop between the pituitary gland and circulating thyroid hormones. When adequate thyroid hormone — particularly T3 — is present, the pituitary reduces TSH production. Medications containing T3, including natural desiccated thyroid, can suppress TSH more easily than T4-only medications like levothyroxine. For this reason, many clinicians evaluate thyroid treatment using Free T3, Free T4, and clinical symptoms, rather than relying solely on TSH.
